ROADMAP
Injury
If the resident sustained an injury or required hospitalization, notify the manager on duty.
The Next Business Day:
- Initiate Root Cause Analysis
- Determine if additional interventions should be implemented
Investigate
Intitate investigation including timeline of the event, resident location, and activities prior to the event.
Notify
Notify the attending physician and responsible party.
Report
Complete the incident report.
Review
Conduct the bedside review--reenact the event if needed. Ensure care plan accurately reflects resident's needs and room arrangement is appropriate. Discuss event with staff and resident.
Assessment
Perform a thorough assessment of the resident, ask the resident what happened.
